At Lake ENT,we want to do all that we can to insure a smooth and efficient visit to any of our three offices. To help make the process easier, we have compiled a list of resources for you to review before, during, and after your visit.
PATIENTS WITH INSURANCE
Improving your hearing can be expensive. Medical insurance companies often provide no coverage for hearing instruments. Our hearing care professionals at Lake ENT will work with you to find a solution within your budget.
We bill all insurance carriers for you when appropriate. For office visits, we file insurance with companies with which we have contracts. If we do not have a contract with your insurance company, you will be expected to pay at time of service and the staff will provide you with two copies of the receipt. One receipt is to file your insurance and one receipt is for your personal records. All co-payments and/or deductibles are due and payable at the time service is provided.
Existing patients, please fill out the forms appropriate to your visit or as instructed by our staff at the time you booked your appointment.
You may either fill out the form online by clicking the link below, print and bring to your appointment or, feel free to print and complete your forms by hand as completely and accurately as possible and bring them to your visit.
Most medical insurance companies provide no coverage for hearing aids however, some health insurance providers, union or employer may provide funds for audiological testing and amplification. Our staff will verify any insurance benefit coverage you may have for hearing aids and bill your insurance when appropriate.

Prepare for your Visit
Our New Patient Form may be downloaded and printed out from your home computer if you would like to fill it out ahead of time and bring with you on your first visit. You may either fill out the form online by clicking the link below, print and bring to your appointment or print and complete your forms by hand as completely and accurately as possible and bring them to your visit.
Make sure to bring your insurance card, form of ID, and any medical information that you may need to keep on file with us. Existing patients, please fill out the forms appropriate to your visit or as instructed by our staff at the time you booked your appointment.
The hearing evaluation process, and hearing aid process are completed during separate appointments. To ensure the best medical advice is provided to you, please be honest and detailed about both your condition and lifestyle so that we may provide best solutions, fitting and results possible.
